This present study seeks to fill this gap by assessing the performance of this test with a community-based sample of Vietnamese American (VA) older adults aged 60+ in the New Orleans area. Therefore, effective early detection of cognitive decline, using appropriately validated test such as the MoCA, is important for this population as the MoCA test in Vietnamese has not been validated with a community-based sample for early MCI screening. 14 These co-morbid factors can put Vietnamese older adults at an increased risk of cognitive decline. In addition, Asian Americans in general are at a particularly high risk for viral hepatitis. 13 VAs also have markedly higher incidences of cervical and liver cancer compared to any other ethnic groups. For instance, compared to other Asian American subgroups, VA women had the highest rates of stroke and dementia. Several medical conditions associated with increased risks of cognitive decline were common among Vietnamese. 9–11 A recent study of the MoCA test in Vietnamese included a very small sample (n=7) of Vietnamese older adults who had been diagnosed with dementia or neurocognitive disorder. 5–7 Research with a community-based sample, where the MoCA and MMSE scores were compared 8 or the internal reliability and construct validity of the MoCA test were examined remains limited. 4 In more recent validations of the MoCA test in languages other than English, the test scores were also compared between groups with and without a clinical diagnosis of cognitive impairment. In many studies, the MoCA test and subtests have primarily been validated against either a clinical diagnosis or the Mini Mental Status Examination (MMSE) score, another tool also widely used for MCI screening. ![]() Koski 3 summarized this literature within the context of cerebrovascular diseases, where cognitive impairment is a common consequence. With a score between 0 and 30 points, the MoCA has the ability to assess multiple cognitive domains 1,2 it has been used in more than 200 countries in different languages, and available for clinical and educational use from Many researchers have validated the MoCA test the vast majority of such studies were with patient populations - those at risk or already diagnosed with cognitive impairments. The Montreal Cognitive Assessment (MoCA) 1 is a simple and user friendly tool widely used for screening mild cognitive impairment (MCI), a transitional stage between normal aging and dementia. The promising performance of a Vietnamese version of the MoCA test seen in this study has important practical implications for the screening of MCI among an important immigrant population in community settings. The study suggests several potential areas for improvement to enhance discriminating power of certain items and sub-tests, including the memory domain. The test was found to be a reliable, and likely valid, instrument for mild cognitive impairment (MCI) screening within our study population. ![]() Our findings indicate good psychometric applicability and internal reliability, as well as construct and criterion validity of this modification of the MoCA test. We also correlated the MoCA score with individual characteristics, including age, sex, and education. Item analyses, and construct and criterion validity analyses were performed. METHODSįorty-eight older adults were interviewed using a Vietnamese version of MoCA. The goal of this study was to assess how the Vietnamese version of the MoCA test performed in a community-based sample of Vietnamese American (VA) older adults, an immigrant population with whom the MoCA test has not been validated. ![]() The patterns vary with the particular etiologic agent and the age of the lesion, as well as with the history and immune competence of the host. The histologic pattern seen on a tissue section captures only one point in time during the life of a lesion, reaction, or reparatory process. These represent expansions of the normal follicular, paracortical, medullary, and sinusal lymph node compartments ( 10). Lymphadenitides and lymphadenopathies tend to exhibit one of four characteristic histologic patterns: follicular, sinusal, diffuse, or mixed ( 9). Specific morphologic features of non-neoplastic lymph node lesions permit them to be distinguished from malignant lesions ( 8). The immune responses may be predominantly of B-cell type, characterized morphologically by either follicular hyperplasia or plasmacytosis, or predominantly of T-cell type, with a characteristic pattern of T-cell hyperplasia ( 7). ![]() Stansfeld distinguishes predominantly inflammatory reactions, caused mainly by bacteria and fungi, and predominantly immune reactions, which are more often the effects of viruses or drugs. The reactions of lymph nodes to foreign substances are diverse and difficult to separate into specific patterns. Some of these are amorphous foreign bodies, which are simply phagocytized and degraded others are antigenic and therefore able to trigger an immune reaction. Thus, a large variety of substances are carried to the lymph nodes. To recall the main etiologic categories, some authors suggest using acronyms like MIAMI ( malignancies, infections, autoimmune, miscellaneous, and iatrogenic) ( 3).Īn essential function of lymph nodes is to filter the lymph drained from their tributary regions. For the etiologic investigation of lymphadenopathies, various algorithms have been proposed, including successive branches in which the various entities are ruled out depending on the response to laboratory tests, medications, and follow-up ( 1). Notwithstanding, in the search for specific etiologic agents in the investigation of lymphadenopathies, the prevailing objective in the majority of cases is to determine the presence or absence of a neoplastic process. Most enlarged lymph nodes involved by RLH or atypical lymphoid hyperplasia do not exhibit morphologic patterns indicative of specific agents, and therefore the cause of RLH is listed as unknown. Still, the cases of RLH in which an etiologic agent is identified represent only a fraction, probably fewer than 10%, of all hyperplastic lymph nodes. In others, characteristic morphologic patterns may suggest an etiologic diagnosis without proving it directly such a situation must be clearly indicated in the final diagnosis. In some cases, specific microorganisms can be detected on regular sections and identified by special stains, in situ hybridization, polymerase chain reaction (PCR), or other procedures, so that an etiologic diagnosis is possible. Additional epidemiologic data to be considered in the diagnosis of lymphadenopathies are environmental factors such as tobacco smoking, occupational exposure to silicon, beryllium or other pneumoconioses, close contact with animals particularly cats, travel to places of endemic infections (see Lymphadenitides), and high-risk sexual behaviors. In a study of 628 lymph node biopsies, 79% were benign lymphadenopathies in patients younger than 30 years of age, 59% in those between 30 and 50 years, and 39% in patients older than 50 years ( 5). In children, most lymphadenopathies are benign, whereas in adults, the probability of malignancy (metastatic carcinoma more so than lymphoma) increases with age ( 4). The age of patients is one of the most important factors in determining the nature of enlarged lymph nodes. In family practice or general medical practice, malignancies represent only about 1.1% of lymph node lesions, whereas at referral centers their frequency is 40% to 60% ( 1, 2, 3). The ratio of non-neoplastic to neoplastic lymph node lesions varies greatly with the kind of medical practice in which they present. Nevertheless, the challenge is to detect those lymph nodes affected by neoplastic processes in order to apply timely and efficient therapeutic measures. ![]() The vast majority of enlarged lymph nodes are non-neoplastic. They represent the reaction of lymphoid tissues to some of the immense variety of intrinsic and environmental antigens. Lymphadenopathies, manifested clinically by the enlargement of lymph nodes, are a common occurrence. Luckily, there’s no shame in starting over, especially if you’re a new player! Is Spider Solitaire a game of skill? Spider Solitaire can quickly get out of hand if you’re not playing your cards right. Now, whether or not you’ll be good at it right away is a different story. Nope! You can be a complete beginner to Solitaire games and still enjoy Spider Solitaire. Does Spider Solitaire require experience to play? Depending on the type of Spider Solitaire you’re playing, you may not use all four suits, but every version of Klondike uses all four suits. Also, in Klondike, you’re building foundations from Ace to King per suit in Spider, you’re building sequences from King to Ace. For one, Spider Solitaire uses two standard 52-card decks, while Klondike only uses one. Klondike, what Americans and Canadians simply call Solitaire, and Spider Solitaire have a lot of differences. What are the differences between Solitaire and Spider Solitaire? A lot of the game is putting pieces together that you’ve been building since the start. One of the best strategies is to build one or two sequences at a time while also stacking together cards that go together. The sooner you complete a sequence, the sooner it’ll be out of the way of other sequences. Always make sure you’ve done everything you can to reveal cards before drawing from the stock.Īnother big point of the game is getting sequences out of play. When these are left unrevealed for too long, stacks will continue to build on top of them, making them harder to reveal. The cards you draw may fill in an area on another side of the board.Īlso, you should try to get the face-down cards on the tableau revealed as soon as possible. What this means is that in order to win Spider Solitaire, it’s best to be careful with where you place cards, but also flexible enough to adjust on the fly. You’ll eventually need to draw cards, and this can cut off sequences if you’re not careful. You can certainly get lucky with your hand, but that luck will run out at some point. Spider Solitaire is all about organization. This becomes a bit tricky the more sequences you complete, as there are fewer cards in play.Īgain, once you’ve completed eight sequences, you win! Importantly, if you want to draw from the stock pile, you must fill any empty space on the board before doing so. In the event that you have an empty column, you can move any card or stack into that space. You’ll need to move these cards to another column if you want to keep building the sequence behind it. As you can see, this will blur the cards above it if it doesn’t add to the sequence. This will add one card to the bottom of each stack. When you’ve run out of available moves with the original tableau (that’s the playing area), you can draw from the stock pile in the bottom right corner. The objective of the game is to create eight sequences, thus using all 104 cards in play. A full sequence is King to Ace, and when this is completed, the sequence is taken off the board. As shown above, a sequence is cards in order, with the highest up top and the lowest at the bottom. Unlike regular Solitaire, you won’t be building foundations instead, you’ll create sequences. ![]() This version of the game uses one suit, but you can find two and four suit variants below. The game uses two decks of cards, meaning you’ll be dealing with 104 cards. You can choose the number of suits - 1, 2, or 4 - to adjust the spider-solitaire card game to your experience and have as much fun as you want. When you begin, you’ll have eight different stacks of cards-that’s where the name “Spider” comes from. ![]() The game features various difficulty levels, stages, seasonal events, and daily challenges. It trains your brain, helps to practice your focus and patience, and enables you to reach higher stages in the game and earn as many points as possible. Spider Solitaire is not only a fun game but also helps to keep your mind active. You can adjust the solitaire difficulty level by playing with 1, 2 or 4 suits. The objective of the game is to stack the cards of a suit in descending order (from King to Ace) to solve free solitaire spider games. The game combines the original rules of one of the most popular classic card games with exciting features to provide an entirely new experience.
